CISSSO is finalizing the preparations for the COVID-19 vaccinations for students aged 12 years and above. All students wishing to receive the vaccination through the school, must pre-register through Clic-Santé before the day of the vaccination.

When entering Clic-Santé’s portal, you must select your child’s school and complete the required sections:
• name and family name of the student (as it appears on the health insurance card)
• name and family name of mother
• date of birth • health card number
• email address (to obtain the vaccination passport)
• phone number
• If your child is 12-13, you must also complete and return (to the school) your signed consent form
